The troubled actress reportedly contacted Holley after arriving at -- and suddenly leaving -- an unapproved facility for her 90-day court mandated treatment. Insiders tell TMZ that Holley demanded a payment schedule in which paychecks would be signed directly over to her before she took Lohan back as a client.
